The radius of that curve looks like it's about 1,000 feet. So obviously one possibility is excessive speed. But there's no intrinsic reason it couldn't be a track defect (other than that there aren't supposed to be track defects), is there? Trains put much more sideways force onto the track on a curve than on a straight stretch unless the curve is perfectly banked for the speed.
