Probable Cause


The National Transportation Safety Board determines that the probable cause of the February 21, 2004, accident was the failure of the engineer of AMLKS-18 to stop as directed by wayside signal because he was asleep, which was induced by his lack of sleep prior to the
accident and his marijuana use. Contributing to the cause of the accident was the failure of the AMLKS-18 conductor to oversee the safe operation of the train because he was also asleep.


Adopted: October 31, 2006

You need more than two people in control of the train. Far better would be one engineer supervising an automatic operation of the train.
Even if the engineer fell asleep, all that would happen is that the train would eventually stop at a red aspect.

Correct. Even the PCT system was in place, but not turned on yet because the FCC had not allotted the needed frequencies.

When they moved the old analog TV channels off VHF for "Digital TV" the original intention was to reserve two of the former VHF band strictly for "public service" use such as police, fire, Amtrak and other public services which needed radio for their own digital needs.

Previous administration to Obama tossed those into the telco sale as well. End result? Police, Fire, etc still don't have enough channels even in the face of "terrorism." :(

Sorry R2 & Joe but these arguments have their limitations. Who exactly in Amtrak should be faulted for not knowing two weeks ago that such a change would be needed specifically at Shore/Frankford? Is there a crystal ball to tell us which of the hundreds of speed drop locations on both the freight and passenger roads will be critical in this matter in the next 10 years? Then how about all the other ways that accidents occur that hindsight will tell us that a minor change to an existing system would have prevented?

Hindsight finger pointing is not a constructive safety engineering method. It's looking over all the potential risks at all the potential locations and somehow coming up with a policy that will hit exactly at the risks that subsequently actually occur that would represent genius. Because covering all of the mere potentialities is not realistically feasible and would actually raise the system complexity to the point of causing new risks.

PRR cab signals/ATC come close, passing a home signal at stop requires a reduction in speed to 15mph as well as an acknowledgement.

Since such an over-speed wreck has never happened here before (the Congressional was not it), no Cost/Benfit analysis could support ATC speed restrictions when ASCES/PTC is months away.

Amtrak says full NEC service will be restored between Philadelphia and other destinations tomorrow morning, beginning with the Train 110 which departs Philadelphia at 5:53 AM, and Train 111, a 5:30 NYP departure. All NEC services, including the Acela Express, will then operate as normally scheduled.

All NEC services between NYP and Washington were cancelled following last Tuesday's deadly derailment of Train 188, a Regional train that overturned on a curve near Frankford Junction.

Amtrak officials said ACSES will also be installed on the northbound track in the derailment area, to govern train movements. Meanwhile, the NTSB and FBI continue to investigate the accident that killed 8 travelers, and injured scores of others, some critically.

It was public record where the proper frequencies went back in the last decade when a certain congress moved them from "public service" allocation to the "investment quality speculation" band. Even sadder, the bankers never used those frequencies at all anyway.

They must have some good people around; they had to wait, then wait some more while NTSB and FBI inspected the scene (100% worth it imho), then get work done in sequence and up to spec, with a s-t ton of alphabet agencies and rulebooks riding their errr, alimentary tract.

While I've never worked on a RR, getting 2 tracks, large poles and megawatt wire up to spec in a few days with all of the headaches involved looks impressive.

In the NY subway they dig up the roadbed, take out all the concrete, old tracks and ties and 3rd rail, pour in new concrete, add new track 3rd rail etc, within a weekend. Not surprised the Amtrak MOW crew restored the line within the weekend.

Yeah, but with NYCT rehabs the new concrete starts crumbling days after the trains run, then the track also go out of line laterally, then larger waves get beaten into them by each passing train. Sooner rather than later they're rebuilding the same tracks again. I've seen this several times on the 7th ave line.

That's because the subways really can't afford to cure concrete properly. Water-cured concrete needs to set for SEVEN DAYS in order to reach 50% of its ultimate strength. Left alone without getting hammered for TWO WEEKS assures that it will pretty much last forever. I can't see shutting down a subway line for two weeks to do things right, thus these results.

Heck, why not. It's a matter of setting expectations.
Some lines can have partial service with one track out. If that doesn't work out, shut a line or segment for 1-2 weeks during the summer, once every 5 or 10 years. Do the rehab well enough that it stays done for a while, the technology exists now. Nearly every line has an alternate of some sort within a mile, which is less distance than us old ones walked to school, under barbed wire through sniper fire ;-).
He||, the MTA could put in a runner's trail covering the OOS distance with pace runners, and charge $17.50/month: TrainIt Fitness. If you are faster than the off-duty crew you get a yard of old rail; slowest runner is conscripted to track work.

Srsly, quite a few people are either out of town or in the home country during the summer, so it's not that big a deal. One could shut 2 miles at a time, but then you'd need a shutdown somewhere on the line every year.

Heh. Too logical. Remember, we're talking about a city that bitches if their train is two minutes late, much less two weeks. But yeah, doing things right is expensive and inconvenient. Better to just screw it all up and shrug and know the train will be running again while the pour is still wet and cold. :(

The derailment of Northeast Regional Train 188 was a terrible tragedy that we are responding to with every resource we have available. The National Transportation Safety Board is leading the investigation to determine the cause of the incident, and Amtrak is providing full cooperation.

With truly heavy hearts, we mourn those who died. Their loss leaves holes in the lives of their families and communities. On behalf of the entire Amtrak family, I offer our sincere sympathies and prayers for them and their loved ones. Amtrak takes full responsibility and deeply apologizes for our role in this tragic event.

We recognize that for everyone onboard the train, including those who suffered injuries, the healing process may be long. Within 24 hours of the incident, Amtrak set up a Family Assistance Center in Philadelphia to work closely with the family of passengers and crew on the train. We are also working with the individuals and families affected by this event to help them with transportation, lodging, and of course, medical bills and funeral expenses.

Amtrak is ever grateful to the City of Philadelphia—its first responders who bravely worked in difficult conditions, including the dark of night, to rescue and provide aid to hundreds; its hospital personnel who went into full alert as patients arrived at emergency rooms; its officials who quickly implemented a response plan; and its citizens who opened their doors to offer assistance.

Although our current focus is on the passengers and employees affected by this incident and the resulting service disruption along the Northeast Corridor, we must also take time to learn from this event. Passenger railroading is at its core about people; the safety of our passengers and employees was, is and always will be our number one priority. Our goal is to fully understand what happened and how we can prevent a similar tragedy from occurring in the future. A "philosophical objection"? Why? If it works and is easily implemented, why not do it wherever it might help improve safety? What is the objection? Engineers are supposed to be observing the speed limits already. Why would doing this "slow down the entire railroad" any more than it needs to be slowed down in these specific spots?

Review all of the chatter on this board in re speed timers all over the subway. The complaints aboput 59-125 stand out. There is not much difference between speed enforcement on the subway and "the big Red subway" (a euphemism for the PRR which I first heard circa 1961)

- FOX news and the right-winged blogs were making much of the engineer's sexual orientation, and some commenting that such people are prone to risk-taking and depression.

- WSJ bashing the Builder and Zephyr

- Larry Kudlow show "merging politics and economics") specifically bashed the Southwest Chief, California Zephyr and the Empire Builder, saying oh so authoritatively that "Nobody rides 'em."

All of it irrelevant and a distraction, showing lazy and stupid journalism.

Wrong! My son rode Southwest Chief awhile back (Mid week & off holiday) & it was packed, sold out. Could have used an extra section.

Larry Kudlow is just a right wing teabagging shill who uses his lack of intelligence & journalistic integrity to hump the republican agenda that encourages "stupid" questions. To hell with reality & the fact that this country needs to maintain the safest possible intercity rail system. The republicans probably wont be convinced until we have folks riding on the roof of trains, Bangla Desh style. I'll issue the first train order, watch for the low wire!

Kudlow once said on his radio program, awhile back, that the reason for gasoline prices to dip (as they did in the mid 2000's) was "Bush's fine economic policies" UGH! Forgot about the basic laws of economics there, Larry??

If you are running nothing more than a bus on rails, then no, a profit is not likely. LION has ridden on these trains too, and they were far from empty. Why, for sake do you think there are so many cars on these trains.

1) Bring back classes of service. Sure we are supposed to be a classless society, but if you put people in box cars then only a certain class (or sort of people) will ride in them.

2) Run "Premium Coach Service", just having good quiet cars with a few amenities will pay for themselves in higher fares.

3) Run sleeper accommodations at a profit. But also bring back some sort of economy sleeper where passengers can get a horizontal accommodation for the night, and then return to the coach for the day time. (Shades of the 1920s eh?) Hey, if you can mulct another $20. from the schlubs in coach, go for it.

4) Dining cars like you really mean it! One or two seating for each of three meals does not cut it. The diner should be open from 5 AM to 9 PM with appropriate menus from simple to complete. The price on meals is too low. $20.00 seems about right for a simple meal, perhaps $30. for the works. ELIMINATE TIPPING ON THE TRAINS: These people are paid a good full union wage with benefits, they do not need tips.

3) There is no evidence that "economy sleepers', once known as Slumbercoaches, made a profit. In any case, Amtrak has no trouble filling their outrageously expensive sleepers as it is. They price for demand. Keeping coach seats open for the day time for these people means dead seat miles in the night time.

I agree 100% about #4 (Diners).
