There were 2 exits to the basement of the Knickerbocker Hotel at the southeast corner of Broadway and 42 St from shuttle track 1 and I think they're both still visible:

Yes, it was like that for at least a few years before they instituted the free transfer to the IND. But I'm thinking that earlier (1960s), the Times Square fare control didn't extend that far west, but instead ended just east of the entrance being discussed. Can anyone confirm that? The entrance presumably was where the tiled sign with arrows showing 8th Ave. to the left and 7th Ave. to the right still exists in the south wall of the passageway. A sign like that would make no sense if the only way to get to 8th Ave. was to pay a fare for the Times Square IRT station and then leave fare control on the 8th Ave. end without riding any trains.

The documents show that the line runs under S4; maybe that is why it was nicknamed so.

The mezzanine at Broadway G actually extends up to an upper level mezzanine above the unused station. It has exits to Messerole and formerly S4 St.

I know that from the #7 train that there is an entrance on 41st Street that is closer to 8th Avenue. Could this be to what the signs are referring to?

Here are pictures of what appear to be the closed entrances on the west side of 6 Ave at 38 St. The first two are of the south side of 38 Street, the door clearly says "SUBWAY" on it, and I doubt that it has anything to do with the chain deli of the same name.

Not disagreeing, but the same type of trap doors exist in many places that were never entrances.

The first two pictures look like standard emergency exits, and may be a sign that there was never a passenger entrance there.

The third is similar to other closed exits, but without labeling it is hard to tell.

Union: Regarding the West 41st Street passageway between 7th Avenue and 8th Avenue - I walked the entire length of West 41st Street today and could find no remaining evidence of the former entrance into the passageway. It was located on the north side of the street to the west of the Nederlander Theatre. There as been a great deal of new construction which must have completely covered the entrance.

There is a video at this link, of the 6th Ave. passageway when it was still open. It shows signs pointing to an exit at "38th St. East side of Sixth Ave." It also appears to show 2 separate staircases to the east side of 6th - one at each corner, perhaps? But no corresponding sign to an exit on the west side. That may help.

Yes. Neighborhood Maps and Apple Maps show an elevator entrance there. This sort of obviates the need for an entrance at 38th.

No it wasn't. I used it only as an exit on the south side of Hillside. It was on the street, facing Queens Blv and I believe nearly flush with the building line. I don't recall whether it had an agent. I used it to pick up electronic parts at Harrison Radio on my way home from junior and senior high school.

It may be that the sealed up exit/entrance at the S/E of City Hall was intended as an access to the lower level platform. As I mentioned in another post, the original plans fpot the BRT/BMT Bway Subway were to have the lower level go through and the upper level be a stub terminal for local trains coming from Queens. The middle tks at cnal were supposed to be connected through as exp tks to points uptown and the Bridge tks would have gone across Canal St to West St. The present arrangement was temporary to get Bkln passengers up Bway prior to the completion of the Montague St tunnel but when plans for the Xtown subway were shelved the arrangement became permanent.

That's about as close to a 100% possibility as I've seen. Most IND subway-to-street-corner exits are right next to the adjacent corner building.

Or they're right at the street corner by the curb. ALA 6th Ave and 14th and 23rd Streets.
