Since the west side 149/Mott platform is coming out of the Harlem R tube, that places it UNDER the east side platform, so no underpinning would have been necessary. At the time the original 149/Mott station was built, there was not even a thought of another subway in that area as yet. As for the Lo-V transfers, the assembled cars would likely have been transferred as you describe, 148 Yd to 135/Lenox, change ends to either just N/O 149/Mott or Jackson Av middle, change ends again back through 1A loop S/O 149/Mott lower to 138/Mott middle and back north to 149 upper or even all the way to Mosholu Yd for storage.
