In a station with a pax in a medical emergency situation, my first instinct, wise or not, would be to pull the cord. This way, train stays until help arrives. With todays phones, not all stations/areas have cell service. A pulled cord will get more attention than a sick passenger. If caught, I'd probably be arrested for criminal tampering if a sick passenger doesn't constitute a true "emergency" even though I might have saved a life.
