Best for baby boomers who still think NYC is a 1980s-era smouldering hellscape: 42nd Street Port Authority
Accessible via the A, C, and E trains

Let me just say something… I get the appeal of hyperbole; it makes whatever you're describing come to life and fuels 99% of BuzzFeed headlines, but such language can also handicap you as a narrator. I mean, would you really “fucking die” for a piece of avocado toast? No. That being said, I’d rather drag my balls through a mile of barbed wire and broken glass than spend more than one nanosecond in the Port Authority bathroom. I’d rather tongue-kiss Tucker Carlson’s thin, wet lips than touch anything in this bathroom. It wasn’t the filthy walls or decidedly haunted baby changing table shoved in the corner, but rather the strangely sweet smell hanging in the air. A filthy ghost lives here and I hate him.

It always puzzled me the sight of liquid hand soap and paper towel dispensers installed, but never stocked with soap and paper towels.

Why are they installed in the first place ?

The only I've ever used is the one at (pre-renovation) Stillwell Ave

Metro-North and LIRR, don't count, right? There, I've used the very clean LIRR ones at Woodside and Forest Hills, and the MN one at 125th St

They have the advantage of on board rooms. I'm sure that makes the station R-R less of a desperation play.

We have to keep in mind that subway bathrooms have been used in the past as places of opportunity for drug addicts to rob an unwary passenger, or for a prostitute to acquire a session with a client, or a drug user to use the drugs just acquired. Frequent visits by police should discourage such opportunistic activities.

When I worked in the Subways, the biggest use for Subway bathrooms was gay sex. You would go into the bathroom of the IRT Grand Central mezzanine, and you would catch rich businessmen from Scarsdale or Cos Cob getting blow jobs by the HIV homeless kids (20s) in the stalls.

One way to discourage misbehavior is to build multiple small donickers, which only one person can occupy at a time.

The last time I used a New York subway restroom was at Main Street Flushing (7) on an extended excursion with another SubChatter. It was tolerable for a quick visit and well used.

The absolute worst public restroom in the world was at the Aurora terminal station of the Chicago Aurora & Elgin Railway, closed in 1957 with the cessation of passenger service. The encrusted filth had not been touched since the station was first opened.
