Its no different than any other system that implements cab signals without wayside signals. If there's a cab signal (CBTC) failure they can run one train per section. On the L it appears the limiting factor is a stretch of 8 or so stations between the tunnel and Myrtle Ave without an interlocking. Adding an extra controlled point or two would mitigate the problem.
