In 2013, a VIA Rail train from Niagara Falls to Toronto (not the Maple Leaf) has 3 engineers in the cab. They all missed an Approach signal to take a slow crossover onto an adjacent track. They took a 15MPH crossover at 67 MPH. It was quite a wreck. All of them were killed. |
