Are you sure it's 67 Av and not 63 Dr? After Alexander's Department Store was built on Queens Blvd between 63 Rd and Junction Blvd in the late '50s, a new subway entrance was built right in front of the store at that location, and it might have been 1962. This is on the north-west corner of the station. There was always a similar entrance on the south-west corner, which had been developed decades before the Alexander's lot, which remained a vacant lot until the late '50s. (This is now a Sears.)

These two entrances do not pass through the mezzanine with the other entrances, and have high turnstiles only with no booths and no cross-over to change directions.
