These things are mechanical devices, they need maintenance and repairs.

And by selling ads on them you can finance their installation and maintenance.

but I presume that they will take up at least 18" of platform space, or will make an island platform 3' narrower.

You can read the original brochure from the Tokyo University staff funded by Kobelco here. Unfortunately their specifications do not give the width, but if the drawing in the middle is to scale, you're looking at around 15cm, or about 6 inches for the platform edge door.

For some that is quite problematic

That makes no sense. If three feet (which again, it isn't, it's a foot) being removed from the platform edge for the purposes of ensuring passengers do not fall into the track area is unacceptable, then the platform itself is too narrow and a perfect place to install a platform screen door.

but they can climb over one of these things too.

And these doors can be installed as full-height barriers it just happens that at this iteration they're designing for a civilized country. In any event, some people will always find their way around whatever barrier is placed in their way. The goal should be to prevent murders such as the one on the 7 train, to keep the track area free of debris swept in off the platforms, and potentially to allow the air conditioning and heating of platforms.

Put up the pipe railings at places where platforms are particularly crowded

These 'pipe railings' are the worst possible solution. You're restricting the stop location of the train to the positions in which the doors will align with the openings in the railing. That means the train has to come in slower, and could restrict the equipment which can be utilized on a given platform.
