I don't think it's a game they are playing. The billing systems they use are all based on regulatory laws and national accepted practices. They follow along with the rules. Doctors seem to not understand how to bill. Codes get bundled or included. They write angry letters. All they had to do was add a 77 modifer, as national coding..(forget the name) and it would be paid. And other simple things.
Some providers don't seem to understand what coinsurance even means. When insurance pays 90% of a bill and you pay 10% or whatever your numbers are. They think it's a denial. Out of network providers think they're going to get paid in full whatever they charge. If that's the case, why even have a network?! And my favorite, the complaints to whatever department of insurance because you asked them to put a valid diagnosis on their bill and that's all, and they think you denied them since they can't read.

Don't ask how I know all of this, but I'd say if you ever hear a complaint, you should hear the full story, because someone is full of it, or they're wrong. I don't think their is any games. They make take the rules in their favour I suppose, but there's no one looking over a single bill trying to nickle and dime it. That's more of department stores and hotels to do.

The Associated Press TALLAHASSEE, Fla. - Florida voters this month approved a three- strikes law unlike any other state's - a measure aimed not at killers and thieves but at doctors who foul up.
The newly approved amendment to the Florida Constitution would automatically revoke the medical license of any doctor hit with three malpractice judgments. The law is backed by doctors' foremost antagonists - lawyers - and the ramifications could be huge.

Legal experts say the measure could let loose a flood of malpractice suits. Doctors say it will scare some physicians away from Florida while forcing others to reach quick malpractice settlements to avoid a "strike" against them.

What about McCains plan? Sounds like something I was saying. He said tax credits, I said give money or make a high deductible HSA attractive. The monthly cost is lower, but if you can get around being scared of the deductible, maybe.

Notice that in politics the debate is over the cost of healthcare, and everyone else is talking about Michael Moore as in the problem is no one can get treatment or something? Still seems like two different things to me.

I have an idea, off the top of my head. I think it feels promising and solves the real issue(which means no one will accept it). It's based on the Florida governors and McCains idea for dealing with hurricanes and disasters. .....
