Two termini have trains reversing in two platform tracks instead of the sidings.

Viz both ends of the Filyovskaya line.

One of these termini handles 34tph with 120m trains. The station has side platforms, and the crossover is unusually nearer the penultimate station that the terminus (although both station are very close together). Trains only dwell for 45secs at this terminus.

Viz Alexandrovsky Sad, which unsurprisingly wasn't designed as a terminal. The real terminal was Izmaylovskaya (three tracks, two island platforms), but that got nicked by the Arbatsko-Pokrovskaya line.

Even if you know there's another train right behind the crowded one in the station, wouldn't you still try to cram on? Its follower, while possibly emptier, will be moving just as slowly, stuck between stations for each of its leader's long dwells.

Well, in the case of a long gap on the Lex Express, the first train should perhaps be sent non-stop from 59th to Brooklyn Bridge, saving the extra hit of the dwell times at GCT and Union Sq.

Yes - the peak frequency on line 4 got hacked back, having been 1m35s for years (and IINM at one stage 1m30s).

What, if anything, is Moscow doing differently?

Each physical line has exactly one route. There is no question of routes having to share tracks with other routes.

This independence results in operational simplicity and better reliability, which allows more frequent services to run.

Still, getting 30tph (per direction) through the scissors crossover at Tokyo station on the Chuo Rapid Line is quite a feat!

The three track terminal that you mentioned, do you happen to know if trains reversed in all three platforms (as at many termini on London Underground), or did some trains reverse in the centre track, whilst others detrained on the outside track, reversed in a turnback track, and re-entered service on the other outside track (as at many termini in Paris, Mexico City, and Corinthians Itaquera in Sao Paulo)?

It must help that unlike Shinjuku, where people sort of fight to get off/on the train, the whole train empties out and then takes new passengers going to the opposite direction.

How many TPH are carried by the local and express tracks of the Queens Blvd IND line? Is it more now after the 63rd St. connection has been completed than it was before the F train was diverted through 63rd. St.? Finally would building 2 new parallel tracks and platforms at Jackson Heights and Forest Hills allow a significant increase in TPH on the Queens Blvd. line or is it still necessary to build a new 2 track Super Express to utilize the unused capacity of the 63rd St. tunnel?

20 local (10 V/10 R)

There has been no overall increase in train service, although the V provides service to Manhattan unavailable when the G ran.

Prior to 63rd St, the E ran 12 TPH and the F 18 TPH.

No idea, although the way the line was extended does make it look suspiciously like the latter.
