Superiors would not do such a thing.The originator of the document is entitled to amplify his material at a later date, and at least in medicine, the date of that entry is also recorded and usually marked as a late entry. For personal notes, I doubt that the process is so formal. IIRC it is a loose leaf book that the officer keeps his notes in, and thus a whole page could be replaced. ROAR |
