I'm not an expert, but on the older cars with the small cabs, you could hear the telephone bell every time the speed restriction dropped. I believe the engineer had to respond in two ways: hit an acknowledgement button and drop the speed appropriately. The system worked so well over decades that I (again non-expert) see no need for Positive Train Control in territory that was so-equipped.

Approaching Jamaica, you could hear the telephone bell ring repeated as the train entered slower and slower trackage.
