Actually it looks like she was lucky enough to fall into the trough between the rails which is a principal characteristic of the type II roadbed that exists in all BMT and IRT dual contract stations ant the entire IND. With that feature, a passenger falling on the tracks and into that trough will be clear enough of the train the injuries will be minimal if any at all other than from the fall itself. In one of the books I read about the subway, there is a story about a sailor who got a bit tipsy and fell to the tracks landing in the trough and fell asleep. Three trains passed over him before he was discovered and brought to the platform and he suffered virtually no injuries at all. Some portions of the dual contract lines and even the IND have had the modified type II installed which has every 5th tie completely across the trough, but there is still enough space between those ties for a passenger who falls to the track to be able to go to clear a train.
