Traditionally, it was Continental Ave after Station Plaza through Forest Hills Gardens (private community. I am not sure of the other info you are looking for.This is exactly what I was looking for. The IND almost always referred to their stations by street and area or line; Queens-Forest Hills, Concourse-205 Street etc. It was then Continental Avenue part that was confusing.
