Strange Metrocard Situation

I went to a vending machine inside the Terminal and put my card into the machine and selected "add value." The machine registered the value as $28.50. I went to add money and the machine rejected the money and displayed "See Agent." I went to the booth at Grand Central and the agent told me my card was voided; all value on the card was taken. I don't have a clue how that happened. Neither did the agent.
