The train operator misunderstood the instructions that she was given. She was reportedly told to pull her train into a siding, change ends and head northbound (on the northbound track). Instead, she changed ends at Canal St and headed north against traffic. Why? Perhaps she didn't understand the instructions. Perhaps she was not as astute as many buffs are about signal placement. Another question was why didn't the C/R. who also has a radio, not pull the emergency cord when he/she realized that the T/O was not following the instructions. From what I found out the crew and 2 managers are sweating this incident out.
