In an ideal situation, police officers can use common sense and determine whether there is a medical situation at hand or not. Also, if a train is stuck in a tunnel somewhere, it is unlikely that there would be a police officer on the train to write a ticket or arrest the passenger. As it is, the officers are supposed to educate about the policy, and are only supposed to issue citations and/or arrest passengers who are belligerent and non-compliant.

The reality is that if you create a rule that says water only, in this day and age where many riders feel entitled to do whatever they want and where they have no feeling of ownership over the system so they don't care what condition it is in for the next rider, it is better to simply ban all food and drink than to leave it up to the discretion of individuals as to what is right or wrong. The only people who seem to have a problem with the current policy are those who consider themselves above the law (see those arrested for eating french fries and candy bars after being asked to cease and desist) or from out of town (who apparently can't figure out why WMATA is cleaner than the trains wherever they came from).
