Terrorist doctrine, both foreign and domestic, is to carry out a primary and secondary attack on the chosen target(s). You should trust me on this, because I've taught this to EMS personnel during inservice training. On 9/11, Al-Qaeda carried out a co-ordinated primary and secondary attack on the WTC. The purpose of all primary attacks is to kill and injure people, create panic, chaos, confusion, and draw emergency services to the scene. The purpose of a secondary attack is to kill and injure further victims, among them as many emergency workers as possible. Both the NYPD and FDNY train their members to always look for, and expect, a second, or even third, attack whenever operating at the scene of a known terrorist incident. That, too, is doctrine.

The FDNY*EMS Operating Guide even addresses the matter specifically, in its Orders 2001-21: All Members Of The Service are reminded to be alert to the probability and possibility of further terrorist activities at or near the scene of emergency operations. These may include but are not limited to: explosive or incendiary devices, use of firearms, missiles, aircraft, watercraft, motor vehicles, or military weapons and appurtenances thereof. Members are reminded that while operating at or near the scene of a known terrorist attack, they are considered targets of ongoing and further terrorist activities.
