It says that the United States shall guarantee to every state in the Union a Republican form of government; doesn't say that applies to the federal government. The federal government is definitely a representative democracy, which is one form of republic but not the only form, which is why the specification is necessary. (All that a republic does is have politicians represent the people, whether they are elected or not. That's why the worst dictatorships have the nerve to call themselves "republics", e.g. Red China, the USSR, ad nauseam.)
