The lack of a uniform, accessible vaccination database is one of many shortcomings. I'm sure some people will use the resulting problems as proof voter registrations are equally insecure.

There is a "NYS Vaccination Passport". It can be downloaded and printed off the web. It contains a QR code. It and some form of ID are supposed to be required for proof of vaccination.

Like most checks, the security of the ID is secondary to how well it is checked. If it's as rigorous as an age check for buying alcohol or tobacco products, Covid will continue to spread.
