You have to consider relocating persons.

People don't grow old in California these days. They move to red states to die.

Same on the east coast. People live in the north then move to the south. They arrive already overweight and diabetic and when they have a bad outcome it counts towards the state they currently live in not where they lived most if their lives.

I think NYC is very healthy. People walk more and probably smoke less. I also think 4 seasons are healthier than brutally long summers.
