You are assuming that he was even aware of the shortage.

Any executive must put in place a system whereby critical information reaches him. That executive must also realize that the critical information is usually bad news. Good news can usually handled by subordinates without bothering the boss. Bad news usually requires an executive decision.

The system that the executive puts in place can have the information bubble up. Sometimes it's necessary for the boss to ask a leading question (can you handle this?) and get a truthful answer. The boss hires his subordinates. If the subordinates don't give him the correct answer it's also the executive's responsibility.

What was that sign on Harry Truman's desk about where the buck stopped?
