The Japanese system is weird and works really well. I don't know exactly the nuances, but they've balanced capitalism with their centralized healthcare such that it boils down to doctors and hospitals competing with each other for patients, since treating a patient = income from the government system (therefore, the more patients the doctor, hospital, etc can see the more money everyone involved makes). A patient sitting in a waiting room is not earning anyone money. If there's only one specialist in an area, and that specialist has a monthlong lead time to see patients, two more practices pop up right next door. There will sometimes be hospitals right across the street from each other, such that you actually *can* pick where you go in an emergency. The better places get more patients, therefore get more money (from the gov't).

I am guessing the reason such does not work in the "west" is (tinfoil hat time) an arrangement between the medical industry and med schools to artificially limit the number of doctors and nurses, causing a perpetual shortage which results in higher costs with less competition. Doctor salaries go up, which makes the value of the med school degree higher, which allows them to charge more in tuition.
